California Assisted Living Waiver Program
The Medicaid Home and Community-Based Services (HCBS) waiver program is authorized as 1915(C) of the Social Security Act. The program permits a State to furnish an array of home and community-based that assist Medicaid beneficiaries to live in the community and avoid institutionalization.
The ALW as it is known is one long term care option for MediCal recipient seniors.
The goal of the ALW is to:
- Facilitate a safe and timely transition of MediCal eligible seniors and persons with disabilities from a Skilled Nursing Facility (SNF) or Nursing Home to a community home-like setting in a Residential Care Facility (RCF), an Adult Residential Care Facility (ARF), or public subsidized housing, utilizing ALW services.
- Offer eligible seniors and persons with disabilities, who reside in the community, but are at risk of being institutionalized, the option of utilizing ALW services to develop a program that will safely meet his/her care needs while continuing to reside in an RCF, ARF, or public subsidized housing.
Today the ALW is operating in Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Joaquin, San Mateo, Santa Clara, and Sonoma Counties.
