Advance Health Care Directive Form

 An advance directive for health care allows you to outline your medical treatment preferences and appoint someone to make decisions on your behalf if you are unable to do so.

What is an Advance Directive?

An advance directive is a legal document that enables you to specify your health care preferences and appoint a health care agent (proxy) to make medical decisions for you if you become incapacitated. It typically includes two main components:

  • Living Will: This part outlines your wishes regarding medical treatments and interventions you do or do not want, especially in end-of-life situations.
  • Health Care Power of Attorney: This designates a trusted person to make health care decisions on your behalf when you are unable to communicate your wishes.
